Location: Escondido, CA
Setting: Hospice Center
Term: 6+ Months
Shift Schedule: Monday-friday | 8AM-5PM
Pay Rate: $110-$115/hour
Astrya Global, a San Diego based medical staffing agency, is hiring a Hospice/Palliative Care Nurse Practitioner for a temporary assignment in Escondido, California. These positions are 26-week contracts with the ability to extend.

QUALIFICATIONS of Hospice/Palliative Care Nurse Practitioner:
Current CA Nurse Practitioner License
Obtain ACHPN certification within one year of hire if not already held
AANP/ANCC certification
DEA License
BLS Certification (American Heart Association)
Maintain a minimum of 3+ years of hospice or palliative care experience
Maintain valid California driver’s license and required auto insurance for travel
Duties/Responsibilities of Hospice/Palliative Care Nurse Practitioner:
Provide home-based hospice and palliative care visits for patients with serious or life-limiting illnesses
Conduct patient assessments and develop individualized care plans focused on symptom management
Perform face-to-face home visits, targeting approximately 45–50 visits per month
Facilitate goals-of-care discussions, advance care planning, and completion of POLST/advance directives
Provide patient and family education related to diagnosis, prognosis, and end-of-life care
Serve as a clinical resource for home aides and support staff
Collaborate with the interdisciplinary team (IDT) and community physicians to coordinate patient care
Participate in IDT meetings and contribute to quality improvement initiatives
Work closely with the Hospice & Palliative Care Medical Director
Document all patient encounters accurately and timely in the My Unity EMR system
Maintain rotating weekend coverage and participate in on-call and 24/7 patient coverage as needed
