Qualifications of a Physical Therapist:

• Active CA Physical Therapist license

• Minimum 2 years' experience in home health (preferred)

• Driver’s License

• BLS Certification

• Auto Insurance

Duties/Responsibilities of a Physical Therapist:

• Perform comprehensive in-home physical therapy evaluations for elderly patients

• Develop and implement individualized treatment plans focused on strength, balance, mobility, and functional independence

• Provide therapy for conditions including post-surgical recovery, stroke, arthritis, neurological disorders, and chronic pain

• Conduct fall risk assessments and implement fall-prevention strategies

• Instruct patients and caregivers on safe mobility, transfers, and use of assistive devices (walkers, canes, wheelchairs)

• Document visits, progress notes, and discharge summaries in compliance with Medicare and home health regulations
